This 5x8s day shift, you will be required to complete admission, recertification, routine, PRN, and death visits. Productivity benchmark 5 to 7 visits per day. Competitive Weekly Pay - $2,400/week for 40 hours Tax-Free Housing & Meal Stipends Travel Reimbursement License & Certification Reimbursement Comprehensive Health Insurance - Medical, dental, and vision Weekly Direct Deposit Referral Bonus Program Full Credentialing & Compliance Support Key Responsibilities of the Travel Home Health Case Manager: Coordinate patient care plans and transitions from admission through discharge Perform utilization review and ensure medical necessity for hospital services Collaborate with interdisciplinary teams, including social workers and providers Educate patients and families on resources, follow-up care, and community support Advocate for optimal health outcomes and efficient use of healthcare resources Maintain accurate documentation and compliance with regulatory standards Requirements of the Travel Home Health Case Manager: Active RN license in Pennsylvania (or compact eligible) BLS certification required Vaild Drivers licence and Vaild Car Insurance required 2 years of recent experience in case management Strong knowledge of discharge planning, insurance authorization, and community resources Strong experience in Hospice and HCHB.
